Or download them\u00a0<a href=\"https:\/\/www.empirevein.com\/en\/patient-center\/#patient-forms\">here<\/a>\u00a0and bring the completed forms to your appointment.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">What to Bring to Your First Appointment<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">We encourage all patients to bring the following to their first office visit:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Insurance card(s) and related information<\/li>\n\n\n\n<li>Referral documents<\/li>\n\n\n\n<li>List of current medications<\/li>\n\n\n\n<li>Co-payment, if applicable<\/li>\n\n\n\n<li>Completed Pre-registration Patient Forms<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Please drink at least 6 to 8 glasses of water before your consultation since we will perform an ultrasound as part of your visit.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Please call our office if there are any changes to your name, address, telephone numbers, or insurance so that we can keep your records up to date.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">If you are unable to keep your appointment, please call our office as soon as possible to reschedule or cancel.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Cancellation Policy:<\/strong> If you are unable to keep your appointment, please call our office as soon as possible to reschedule or cancel.
