As more people use these medications, questions about GLP-1 and varicose veins are becoming more common. Can weight loss reduce pressure on the veins? Could rapid changes in body weight make existing veins more noticeable? Do the medications affect the risk of a blood clot? The answers are not always simple.

Can GLP-1 Medications Help or Harm Your Veins?

Weight loss may support healthier circulation and reduce some risk factors for vein disease. GLP-1 medications may affect vein health indirectly through weight loss, improved blood sugar control, and changes in circulation. However, they cannot repair damaged vein valves or make existing varicose veins disappear.

How GLP-1 Medications Affect Your Body

GLP-1 medications act like a hormone your body naturally produces after eating. They help regulate blood sugar, slow digestion, and reduce appetite. This can lead to significant weight loss for some people. These medicines include semaglutide and tirzepatide. You may recognize brand names such as Ozempic®, Wegovy®, and Zepbound®. [3]

Losing excess weight may reduce pressure on the veins in your legs. Obesity is a known risk factor for varicose veins because extra body weight can make it harder for blood to travel upward toward the heart. It can also contribute to inflammation, limited mobility, diabetes, high blood pressure, and other conditions that affect vascular health.

For some people, GLP-1 medications may also provide cardiovascular benefits beyond weight loss. In the 2025 STRIDE trial, semaglutide improved walking distance, symptoms, and quality of life in adults with type 2 diabetes and peripheral artery disease (PAD) over 52 weeks compared to a placebo. These gains were seen across starting weight and blood sugar levels, suggesting benefits beyond weight loss or blood sugar control alone. [2]

PAD affects your arteries, while varicose veins affect your veins. High blood sugar from diabetes can damage blood vessels over time and make it easier for plaque to build up in the arteries, increasing the risk of PAD. While GLP-1 medications don’t treat varicose veins directly, losing weight and controlling blood sugar helps support your overall circulation.

Could GLP-1 Medications Affect Arthritis Symptoms?

GLP-1s are also being evaluated for their effects on joint health. In the 68-week STEP 9 trial, semaglutide reduced knee pain and improved physical function in adults with obesity and knee osteoarthritis. Less joint pain may make walking easier. When you walk, your calf muscles contract and help push blood back toward your heart, improving leg circulation. However, the STEP 9 study did not assess blood flow, and GLP-1 medications are not approved for arthritis. [5]

Can Weight Loss Improve Varicose Veins?

The connection between weight loss and varicose veins mainly involves pressure, movement, and symptom management.

Reaching a healthier weight may reduce pressure on the veins and make physical activity easier. Walking activates the calf muscles, which help push blood out of the legs and toward the heart. Weight loss may therefore help reduce swelling, heaviness, fatigue, and discomfort in some people.

Weight loss may help prevent symptoms from getting worse and lower the risk of new veins developing. However, it usually does not eliminate varicose veins that are already present.

Varicose veins form when vein walls or one-way valves become weak or damaged. Blood then pools in the vein, causing it to enlarge, twist, or bulge. Once this damage has occurred, losing weight cannot restore normal valve function.

The relationship between varicose veins and weight loss can also involve appearance. After significant weight loss, there may be less tissue covering the veins. As a result, existing veins may look more prominent even if the underlying vein condition has not worsened.

People who notice varicose veins after weight loss should not assume the medication created them. The veins may have already been present but were less visible. A vascular specialist can use an ultrasound to check for venous insufficiency and determine whether treatment is appropriate.

Do GLP-1 Medications Increase Blood Clot Risk?

So, does Ozempic cause DVT? The short answer is that research hasn’t proven a direct link, and the full picture is still evolving. Concerns around deep vein thrombosis (DVT), a blood clot that forms in a deep vein, typically in the legs, are understandable. When a clot develops in a deep leg vein, it can break loose and travel to the lungs, causing a pulmonary embolism, which is a very serious condition.

A 2025 systematic review examined 39 randomized trials involving more than 70,000 participants. Overall, researchers did not find a major increase in blood clots. However, a closer look showed a slightly higher rate of DVT among people taking GLP-1 medications. They noted that these events are still rare and that more research is needed. [4]

Dehydration (from nausea, vomiting, or diarrhea) and inactivity (from fatigue or illness) increase clot risk by allowing blood to pool. Other key DVT risk factors include obesity, smoking, past clots, surgery, cancer, pregnancy, and underlying medical conditions.

Contact a medical professional promptly if you develop new swelling, warmth, redness, or pain in one leg. Chest pain, sudden shortness of breath, coughing up blood, or fainting requires emergency medical care.

What to Know About Spider Veins After Weight Loss

Spider veins are smaller than varicose veins. They usually appear as thin red, blue, or purple lines near the skin’s surface.

In most cases, spider veins do not usually go away with weight loss. Weight management may improve circulation and reduce pressure on the legs, but it does not remove blood vessels that have already become visible.

Spider veins may appear more noticeable after rapid or significant weight loss because there is less tissue beneath the skin. Genetics, age, pregnancy, hormones, prolonged standing, and previous vein damage can also contribute to their development.

Some spider veins are mainly a cosmetic concern. Others occur alongside venous insufficiency. This is more likely when they are accompanied by aching, swelling, heaviness, skin changes, or larger varicose veins.

When to See a Vein Specialist About Your Symptoms

Schedule an evaluation if you have:

Aching or cramping

Heaviness

Swelling

Fatigue

Restless legs

Skin changes

Open wounds or sores

Visible varicose or spider veins on the legs

These symptoms can indicate venous insufficiency, even when the veins do not look too bad. A vein specialist can examine your legs and use ultrasound imaging to evaluate blood flow and valve function.

At Empire Vein & Vascular Specialists, our board-certified specialists offer a Free Vein Screening regardless of your insurance status.
